Field Nursing


Program Overview
The primary focus of the work of the Field Nursing staff is individual case management and health interventions and education with clients.

Guiding Principles
For each population served, we seek to design systems of care that help clients acheive improved health outcomes consistent with their personal goals, community goals, and specific standards.  Our practice is guided by professional and program-specific standards, and incorporates the eight core competencies of Public Health Nurses in California:

  • Nurse-client reationship and management of client health/illness status
  • Community assessment, assurance and advocacy in population-based services
  • Environmental assessment and intervention
  • Professional role - coaching functions
  • Monitoring and ensuring the quality of health care practice
  • Education of clients, families and the community
  • Epidemiology

Services Provided
Case Managment Services: Public Health Nurses (PHNs), together with Community Workers, provide case management services in the clinic, home, community, office and school to promote healthy living and prevent illness.  Their goal is to assist patients with education, guidance, support and access to community resouces.  An effort is made to provide culturally-sensitive services in the client's native language.

There are two County-run clinics (North County and Coastside) that have a PHN stationed on-site.
